Non-hormonal Management of Vasomotor Symptoms

Abstract
Vasomotor symptoms are the most common symptoms during the perimenopausal transition and menopause. They are experienced by up to 75% of menopausal women. Hot flashes generally persist for 1 to 2 years and can last for several years after menopause. Systemic estrogen therapy is the most effectives treatment strategy for vasomotor symptoms and has been used for nearly 60 years. When menopausal women do not want to take hormone therapy or when it is contraindicated, there are several therapeutic options for hot flashes. For mild hot flashes, lifestyle modification such as lowering body temperature, weight control, exercise and relaxation techniques may have some effects without complication. Nonprescription remedies such as isoflavone, black cohosh and vitamin E have not shown sufficient efficacy in clinical trials. However, short-term application of these remedies might not be associated with serious adverse effects. The antidepressant paroxetine, fluoxetine, and venlafaxine and the anticonvulsant gabapentin have shown some efficacy for moderate to severe vasomotor symptoms. Regardless of treatment options, clinicians should evaluate the symptoms of the patients periodically.
